Uganda Traveller Health Declaration form

Uganda have introduced a mandatory public health measure implemented to support the monitoring and prevention of infectious diseases. The Traveller Health Declaration Form is required for travellers entering Uganda and must be filled in prior to arrival. All information submitted is treated as confidential.


The form is quick to complete (approximately 2 minutes) and is used by Ugandan health authorities to support public health monitoring and prevent the spread of infectious diseases.


Travellers will be required to provide:

  • Relevant travel details (including date of arrival, port of entry, flight or vehicle number, and countries visited within the previous 21 days)

  • Personal information as stated on their passport

  • Details of their stay in Uganda (including address and duration


Why is this form needed?

  • Disease prevention and control: It enables health authorities to monitor travellers for potential exposure to infectious diseases and take appropriate action if necessary.

  • Early detection and response: By collecting travel history (including countries visited in the last 21 days), authorities can quickly identify higher-risk travellers and respond promptly to protect public health.

  • Contact tracing capability: The information provided allows authorities to contact travellers if a health risk is identified after entry, helping to limit any potential spread.

  • Entry screening efficiency: Completing the form in advance helps streamline arrival procedures at points of entry (such as Entebbe Airport), reducing delays and ensuring smoother processing


Once you have completed and submitted the Uganda Traveller Health Declaration form, the following typically happens:

1. Submission confirmation

  • You will receive a confirmation (usually on-screen) that your form has been successfully submitted.

  • In some cases, you may also receive a confirmation message or reference/QR code to present on arrival.


2. Use at point of entry

  • On arrival in Uganda (e.g. Entebbe Airport), health officials may:

    • Verify your submission (via name, passport, or QR/reference code)

    • Review your declared travel and health information

  • This helps speed up entry screening procedures.
